Risk Assessment
The Risk Assessment module lets you conduct structured, activity-based risk evaluations within a project. You identify activities, assign hazards to each activity, evaluate likelihood and severity, document control measures, and track residual risk — all within the app.
Note
Risk Assessment is available to users with Standard, Key User, and Admin roles. Free-tier users have read-only access.
Accessing Risk Assessment
Navigate to the Findings tab and select the Risk Assessment tab, or access it from the project menu. Risk assessments are scoped to the current project.
Activity-Based Assessment
Risk assessments are structured around activities — the tasks or operations being evaluated. For each activity you:
- Select or create an activity from the activity list
- Add one or more hazards associated with that activity
- For each hazard, evaluate likelihood and severity
- Record existing control measures and proposed additional controls
Risk Scoring
Risk is calculated automatically:
Risk Score = Likelihood × Severity
Both likelihood and severity use a 1–5 scale. The resulting score (1–25) maps to four risk levels:

| 1–5 | Low | Green |
| 6–12 | Medium | Yellow/Amber |
| 13–19 | High | Orange |
| 20–25 | Critical | Red |
Control Measures
For each hazard you can document:
- Existing controls: measures already in place that reduce the risk
- Additional controls: proposed actions to further reduce residual risk
After applying controls, you can re-evaluate the residual likelihood and severity to confirm the reduced risk score.
Integration with Findings
Risk assessment items can be linked to specific findings. This creates a direct connection between a documented hazard and the corrective action or inspection finding associated with resolving it.
Quick Filters and Grouping
Use the quick filter bar to filter risk items by activity category or risk level. Switch between group-by modes to organize the list by activity, risk level, or status, making it easier to focus on high-priority items.
