Incident Reporting

The Incident Reporting module takes an incident from the first minute to the last lesson: report in under a minute, rate the risk on your company's matrix, investigate to the root cause, drive corrective actions along the hierarchy of controls — and, for German operations, file the statutory Unfallanzeige (U 1000) without leaving the app.

Report in a minute

One form, nine classifications — Fatality, Lost Time Case, Medical Treatment Case, Restricted Work Case, First Aid Case, Unsafe Condition, Unsafe Act, Environmental Incident, Positive Observation. Add photos, dictate the description instead of typing it, pick the location from your site's zone hierarchy, and save. The right people are notified automatically.

A QR on the wall is enough

Print the incident report QR code and post it in the workplace. Anyone who scans it lands on a report form — and with external reporting enabled, no account is required: the report goes straight to the safety team. A "require sign-in" switch keeps the QR restricted to signed-in users where that's preferred.

Rate the risk

Rate the actual severity of what happened and the potential of what could have happened, side by side, on your company's risk matrix — one tap on the cell where likelihood meets severity. High-risk incidents surface instantly in lists, filters, and dashboards.

QuickInvestigate

The investigation tab structures the real work as a five-why chain across four lenses — Design, Environment, People, Management. An AI investigator can draft the chain from your photos and description; every AI-generated cause is marked for human review. Flag the root cause and categorise it — a root cause categorised as inadequate training automatically creates a training recommendation for the involved workers.

Corrective actions on the hierarchy of controls

Turn causes into corrective actions — assigned, dated, prioritised, and typed along the hierarchy of controls: Elimination, Substitution, Engineering Controls, Administrative Controls, PPE. Each action stays linked to its incident, and the incident cannot be closed while any linked action is still open.

Unfallanzeige (U 1000) built in

For companies operating under German law, a guided five-step wizard produces the statutory accident report:

• § 193 SGB VII guidance — the wizard tells you when an incident is reportable (fatal or more than 3 days of incapacity) and the 3-day deadline that applies
• Carrier selection across the real BGs and Unfallkassen, with your company number stored once in company settings
• Incident data — course of events, injured body parts, location, times — is pre-filled from the incident
• One tap creates the official U 1000 PDF, with a deep link to the insurer's portal and a tracked draft → PDF created → submitted status

Close with the whole story

When the root cause is marked and every action is done, Finish Investigation closes the incident — photos, ratings, whys, actions, and the full change history stay preserved and exportable.

Every report sharpens the picture

Incident KPIs feed your safety dashboards: open incidents, days lost, LTC counts, trends by classification, and frequency rates based on your recorded worked hours — with the rate basis (per 1,000,000 or 200,000 hours) configurable to match EU or OSHA conventions.
