QuickInspect: Report Your First Incident
Anyone should be able to report an incident, an unsafe condition or a positive observation quickly. QuickInspect offers two ways: the full form in the app, and a QR code that also works for people without an account.
In the App
- Open Incidents from the bottom bar (or the Incidents tile on the home screen).
- Tap + (or Report an incident if the list is still empty).
- Fill in the required fields:
- Title and Description -- what happened.
- Classification -- Fatality, Lost Time Case, Medical Treatment Case, Restricted Work Case, First Aid Case, Unsafe Condition, Unsafe Act, Environmental Incident or Positive Observation.
- Location -- where it happened.
- Injured body part -- only for classifications that involve an injury.
- Add photos if you have them. On paid seats, Analyze with AI can draft the description and a risk rating from a photo for you to check.
- Tap the save icon at the top right. The app confirms with Incident saved.
Tip
Report first, complete later. The first report only needs the essentials -- the investigation, risk ratings and actions are added afterwards by the people responsible.
Through a QR Code
Put a QR code on a notice board, in the canteen or at the site entrance so anyone can report on the spot:
- On the Incidents list, tap the Incident QR Code icon in the top bar.
- Decide whether Require sign-in should be on:
- On -- people log in and get the full in-app form.
- Off -- anyone can report through a short public form, without an account. This needs the company setting Allow external incident reporting.
- Print QR Code, Copy Link or Share it.
The public form asks for a title and what happened. Everything else is optional: the type of incident, where and when (pre-filled with the current time), the reporter's name and email, and up to three photos. Submit Report sends it in.
What Happens Next
- Colleagues in the company and project are notified of the new incident.
- If the incident belongs to a department or contractor with responsible persons, they are notified and assigned.
- The incident is then investigated on its Investigation tab (five-why analysis and root cause, with AI help if enabled), and corrective actions are added on its Actions tab.
- Actual and potential risk ratings are set by the people handling the incident -- they are not part of the first report.
- The incident can only be closed once a root cause is recorded and all its actions are closed.
